Initial Comments:
An unannounced, onsite follow-up survey completed February 5, 2019, found Commonhealth Orthopedic Physical Therapy had corrected the deficiencies cited under 42 CFR, Part 485.727, Subpart H, Conditions of Participation for Clinics, Rehabilitation Agencies, and Public Health Agencies as Providers of Outpatient Physical Therapy and Speech-Language Pathology Services - Emergency Preparedness. The deficiencies were originally cited as a result of a Medicare recertification survey completed December 27, 2018.

An onsite unannounced Medicare recertification survey completed February 5, 2019, found that, Commonhealth Orthopedic Physical Therapy had corrected the condition level and standard level deficiencies cited and was in compliance with the requirements of 42 CFR, Part 485, Subpart H, Conditions of Participation for Clinics, Rehabilitation Agencies, and Public Health Agencies as Providers of Outpatient Physical Therapy and Speech-Language Pathology Services. As a result of the survey, the condition level citations have been lifted as evidenced by full implementation of the approved plan of correction.
The deficiencies were originally cited as a result of a Medicare recertification survey completed December 27, 2018.
